Before Lord Voldemort's first defeat the diary was passed to Death Eater Lucius Malfoy. Malfoy then slipped the diary, into Ginny Weasley's cauldron at Flourish and Blotts. The diary used dark magic to force Ginny to re-open the Chamber of Secrets, but it was destroyed by Harry Potter by stabbing it with a Basilisk's Fang.
